How to Read Your Testosterone Labs (Without Getting Misled)

The Headline Number Is Only the Headline

When the lab returns your results, the first thing you see is usually a single number labeled "Testosterone, Total." You look at it, compare it to the printed reference range, and leave with a feeling. If it is high, you feel reassured. If it is low, you feel alarmed. Most men stop there, and most men walk away with the wrong conclusion.

The reason is that the printed reference range is a population statistic, not a target. A 300–1,000 ng/dL "normal" range describes what 95 percent of adult men happen to have. It says very little about what is optimal for a 42-year-old who lifts four days a week and has not felt like himself for two years. A man sitting at 380 ng/dL of total testosterone with the symptomatic profile of low T is inside the lab's "normal" band and is being given a clean bill of health he does not actually have. To know what is going on, you have to read the other three numbers on the panel.

The Four Numbers That Actually Matter

A useful panel measures four hormones, not one. Each one tells you something the others cannot, and each one is needed to make a real interpretation.

  • Total testosterone — Every circulating testosterone molecule, bound and unbound. The headline number. Below 300 ng/dL is clinically hypogonadal by most guidelines. The 300–450 ng/dL band shows up in the lab's reference range as "normal" and is the band where most symptomatic men land before they get a fuller panel.
  • Free testosterone — The fraction of total testosterone not bound to SHBG and therefore available to receptors. This is often the more diagnostic number. A man with 450 ng/dL of total T but 6 pg/mL of free T has the same hormonal situation as a man with 300 ng/dL of total T and equivalent free T. The two are functionally the same.
  • SHBG (Sex Hormone Binding Globulin) — The protein that binds testosterone and renders it biologically inactive. Rising SHBG is the most common explanation for the "normal total T, symptomatic" pattern. It is elevated in endurance athletes, in older men, and in men with low body fat and high fiber intake. A high SHBG number on the panel is the missing piece of the puzzle for an enormous number of men.
  • Estradiol (E2) — The form of estrogen produced when testosterone aromatizes. Men need estradiol in controlled amounts. High estradiol (above 40 pg/mL) suggests excess aromatization and is associated with water retention, mood swings, and the body-fat feedback loop that worsens low T. Low estradiol affects libido, joint comfort, and bone density. The E2 number should be on the panel any time testosterone is.

What the Numbers Mean Together

Reading the four together, not individually, is where the panel becomes useful.

If total T is below 300 ng/dL, the diagnosis is straightforward — clinically low testosterone regardless of how you feel, and a TRT consultation is warranted.

If total T is between 300 and 450 ng/dL and you have symptoms, look at free T and SHBG. Most symptomatic men in this band have a meaningful free T deficit driven by elevated SHBG. The total number is "normal," but the available number is not.

If total T is above 450 ng/dL with symptoms, testosterone is unlikely to be the primary cause. Sleep apnea, hypothyroidism, depression, vitamin D deficiency, and overtraining all produce the same cluster of symptoms with normal total T. Rule those out before chasing hormones.

If SHBG is high and free T is low despite normal total T, the issue is available testosterone, not produced testosterone. Some TRT protocols address this directly; physician guidance matters here, because the wrong protocol can further elevate SHBG.

If estradiol is out of range in either direction, the panel is not finished being read. E2 affects TRT decisions downstream and matters for cardiovascular and bone health immediately.

How to Get a Panel You Can Actually Read

Draw in the morning, between 7 and 10 am, after a normal night of sleep, two days removed from a hard training session. Most GP labs will only run total T by default — insist on the full four-marker panel, or order it yourself from an at-home provider. Bring the results to a physician who reads panels in the context of symptoms and history, not just against the printed reference range. The same numbers mean different things for a 26-year-old athlete and a 48-year-old executive, and the interpretation needs to reflect that.
